ダイエットで痩せる原則は食事療法と運動療法ですが、

ライフスタイルの欧米化に伴って、
わが国の生活習慣病患者は増加の一途をたどっており、
その主要なリスクファクターである肥満を
良くするためにダイエットで痩せることが重要になっています。

日本肥満学会では、
痩せるためのダイエットで減量することにより
肥満に伴う合併症が改善すると考えられる状態、
あるいは臨床的にその合併が予測される場合（内臓脂肪型肥満）には、
治療が必要となる状態（疾病単位）として肥満症と定義しています。

肥満症治療の原則は食事療法と運動療法ですが、
高度肥満や運動療法の困難な場合には薬物療法が必要となります。

肥満は遺伝素因と環境因子の
複雑な相互作用により発症する代表的な多因子疾患です。

今までは、分子レベルで調べることが困難でした。

1994年末に遺伝性肥満ob/obマウスの原因遺伝子として
レプチンが発見されて以来、
エネルギー代謝調節の分子機構に関する知識が集積されてきており、
ダイエットで痩せるために役立つ、
多くの新しい抗肥満薬症薬が提案されています。

肥満は摂取エネルギーと消費エネルギーバランスが
くずれることにより発症するため、
摂取エネルギー（食欲）の減少
あるいは消費エネルギー（熱産生）の増大をもたらす分子群が、
ダイエットで痩せるために役立つ、
抗肥満薬症薬と考えられています。

実際に、肥満症の治療薬は、
①中枢性食欲抑制剤
（ノルアドレナリン作動薬、セロトニン作動薬、ノルアドレナリン・セロトニン作動薬）、②熱産生促進薬（ノルアドレナリン作動薬、β3-アドレナリン受容体作動薬）、
③消化吸収阻害薬（リパーゼ阻害薬）
④その他、
のように分類することができます。

現在日本で使用可能な抗肥満薬はマジンドールですが、
この薬を使うためには、
病院に行って医師に、
肥満を治療することが必要だと判断してもらい、
ダイエットで痩せるためには、
食事療法と運動療法だけでなく、
薬の助けも必要だと診断してもらう必要があります。
